The First Visit
- Please complete the necessary forms and plan to arrive at least 15 minutes early to review all documentation.
- Please bring a valid photo ID and insurance card
- Worker’s Compensation cases - please provide the name of the case manager, telephone number and your claim number
- Personal Injury cases – please provide the name of the attorney, claim number, insurance company and telephone number
- Please arrange to have your medical records sent in advance or bring a copy with you.
- Bring all medications you are taking to the appointment. We will gladly assist you in maintaining an updated medication list.
- We will also need information about your insurance coverage. If any of this information changes between visits, please let us know so that we can update your records.
